Recommended Items / Documents to bring to your Tax Preparation Appointment

  • A copy of last year’s taxes if available

  • Valid photo ID for all adults on the return

  • Social Security Cards or ITIN letters for everyone on the return

  • Information regarding your California Middle-Class Tax Refund (1099-MISC)

  • All wage and earning statements: Form W-2, W2-G, 1099-R, 1099-MISC, 1099-NEC

  • Form 1095-A & Form 3895 health insurance marketplace information (if you had medical insurance through the marketplace / Covered California)

  • Form 1095-B or 1095-C for Medi-Cal or medical insurance through an employer

  • Form 1098-T (If you were a student)

  • Form 1099-INT / 1099-DIV - Interest and Dividend statements

  • Total amounts paid to childcare providers and their tax ID numbers

  • Routing and account numbers if you want to receive a direct deposit

  • If you are married and filing jointly, your spouse must be present.
